Incidents & Corrective Actions
Incidents is the safety record: what happened, how bad it was, what was done at once, why it happened and what will stop it happening again. Corrective actions raised anywhere in the safety management system land in one register and pass through a verification gate before they can close.
The screen
Three tabs: Incidents, Risk Register, Risk Matrix (the 5×5).
On Incidents, the Safety triangle sits beside Lifecycle & root cause: a clickable lifecycle bar, a count of overdue investigations and the root-cause mix. Filter by search, vessel, status and type, or by the chips High severity and Regulatory.
Expand an incident for its description, immediate actions, root cause, contributing factors, lessons learned, corrective actions, metadata, weather, injury and any environmental release, plus 5 Whys, witness statements and regulatory filings (pending, submitted, acknowledged, closed).
Reporting an incident
Report Incident needs the vessel, date, location, type, severity (1 to 5), a description and who reported it. Everything else can be added as the investigation proceeds. Add Risk does the same for the risk register.
Corrective actions
The Corrective Actions page is the fleet CAPA register. Its Verification gate has four stages — Open, In progress, Awaiting verification, Verified — an ageing bar from "more than 30 days to due" through "overdue 90+", and a Recurrence signal.
Raise one from an incident (Raise CA) and move it with Start work, Complete…, Extend due…, Escalate…, Verify effectiveness…, Verification failed — reopen…, Mark as recurrence…
What it will and won't do
- Every change of state on a corrective action asks you to write something. Completing needs a note and evidence. Extending a due date needs a reason and keeps the old date. Failing verification needs a reason. Verifying effectiveness is a separate step after completion, never the completion tick itself.
- Recurrence is tagged against ISM 9.2.
- Deleting an incident, a risk or an action asks you to confirm and warns it cannot be undone.
Incident statuses: Draft, Open, Under Investigation, Corrective Action, Closed. Types: Near Miss, LTI, MTC, RWC, FAC, Property Damage, Environmental, Security, Collision, Grounding, Fire, Explosion.
