Defect Register

The Defect Register is where anything wrong with a ship is written down: who found it, how serious it is, what is being done and when it is due. It shows the fleet's open defects by severity and age, so the oldest serious ones cannot hide.

The screen

The top band shows the states that stop a ship trading: DETAINED, MAJOR NC, CoC OVERDUE, CoC DUE. Below it the Escalation matrix — severity down the side, age across the top — where every cell is clickable and filters the list. Alongside: Fleet hotspots and Inflow vs outflow.

Filter by search, status, severity and vessel, and switch between List and Board. Expand a defect for its description, resolution and regulatory identity, plus notes.

Reporting a defect

Report Defect asks for a description, the system and component, who reported it and their role, photos (upload or straight from the camera on a phone), and an equipment link. If the defect is being deferred, you record the reason, a risk assessment, the compensating measures, who authorised it, and any external instrument or class reference.

From defect to action

Each defect has two bridges: open a corrective work order pre-filled from the defect, or promote it to a tracked corrective action in the CAPA register. Once promoted, the defect shows its corrective-action badge instead of the promote button.

Export CSV downloads what is currently in scope with its number, vessel, system, component, description, severity, status, dates and reporter.

What it will and won't do

  • The escalation matrix counts open defects only, so its numbers are smaller than the full list on purpose.
  • Delete asks you to confirm the defect number.
  • The hard states carry their meaning in a tooltip: a detention means the ship may not sail until re-inspected; a class memorandum is informational with no due date; a vetting observation is a commercial consequence, not a statutory one.

Severity: Critical, Major, Minor, Observation. Status: Open, In Progress, Pending Parts, Pending Class, Deferred, Resolved, Closed. Source: Crew Report, PSC Inspection, Class Survey, Internal Audit, Vetting, Condition Monitoring.
